The Bridge at Fair Park

The applicant must meet the service and the medical criteria to be eligible for Day Activity and Health Services. The applicant must

1. Be Medicaid eligible or meet the income and resource guidelines
2.  Meet the minimum functional need criteria
3. Have a medical diagnosis, a related functional disability, and physician’s orders requiring care, monitoring, or intervention by a licensed vocational nurse or a registered nurse
